About the Role:

The Hospice Visit Licensed Practical Nurse (LPN) working evening shifts from Wednesday through Sunday plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses. This position focuses on providing skilled nursing care, symptom management, and emotional support to patients and their families in their homes or care facilities during the evening hours. The LPN will collaborate closely with interdisciplinary hospice teams to ensure that care plans are effectively implemented and adjusted according to patient needs. By maintaining accurate clinical documentation and communicating changes in patient status promptly, the LPN helps to optimize quality of life and comfort for hospice patients. Ultimately, this role is essential in supporting the dignity and well-being of patients during their end-of-life journey while providing reassurance and guidance to their loved ones.

Minimum Qualifications:

  • Current and valid Licensed Practical Nurse (LPN) license in the state of practice within the United States.
  • Minimum of one year of clinical nursing experience, preferably in hospice, palliative care, or home health settings.
  • Basic Life Support (BLS) certification from an accredited organization.
  • Ability to work independently during evening shifts with strong time management and organizational skills.
  • Reliable transportation and willingness to travel to patient homes or care facilities within the assigned geographic area.

Preferred Qualifications:

  • Previous hospice or palliative care experience.
  • Certification in Hospice and Palliative Nursing (CHPN) or similar specialty certification.
  • Experience with electronic medical records (EMR) systems and documentation.
  • Strong interpersonal and communication skills tailored to sensitive end-of-life care situations.
  • Familiarity with pain and symptom management protocols specific to hospice care.

Responsibilities:

  • Conduct comprehensive nursing assessments and monitor patients’ physical and emotional conditions during evening visits.
  • Administer medications, treatments, and symptom management interventions as prescribed by physicians and hospice protocols.
  • Provide education and emotional support to patients and family members regarding disease progression, care techniques, and available resources.
  • Collaborate with interdisciplinary team members including physicians, social workers, and chaplains to coordinate holistic care plans.
  • Document all nursing care, patient responses, and any changes in condition accurately and timely in compliance with regulatory standards.
  • Respond promptly to patient and family concerns or emergencies during scheduled visits, escalating issues as necessary.
  • Ensure adherence to infection control policies and maintain patient safety during all visits.
